Octave Solutions - PrognoCIS EMR Lead Referral Terms and Conditions

1) Submitting a medical practice or doctor as a lead without knowledge of the medical practice or doctor can be terms for dismissal from this program. This is to avoid people from simply cutting and pasting doctor listings from yellowpages etc..., into our database.

2) Octave Solutions reserves the right to change this program at any time. However, we will work diligently to ensure our referral partners receive published compensation for their efforts.

3) A qualified lead is a prospect who does not have EMR, or is not happy with their current EMR and may consider PrognoCIS EMR.

4) A sale is results when the lead purchases PrognoCIS EMR, either ASP or Hosted, from Octave Solutions.

5) The referral amount is generally $250.00 but may be more depending on the size of the opportunity. There is no maximum to the number of leads you can reffer Octave Solutions!

6) Payment to the referrer is made approximately 30 days after the sale is complete.
